Sports Physical Paperwork
- In order to tryout, a Sports Physical is needed (if you have not had one yet this school year). You can get the form here.
- Complete Medical Update Form:If you have already had a physical this year, please complete the Medical Update Form here.
- Important Note:If you do not have your paperwork submitted to a nurse by Monday, March 4th, you will NOT be able to try-out for the team.

- Check-in with the coach in the Athletic Center assigned to your grade
- Wear athletic gear and cleats/sneakers
- Mouthguard is recommended
- Be warmed up with athletic gear on before practice start time
- Players will be invited back after each practice
- Lists will be posted on Google Classroom after each practice
- Sophomore & Junior Invites: March 6th posted by 5pm
- Freshman & Senior Invites: March 8th posted by 5pm
- Final Team Selection: March 11th posted by 7pm
- If your ID Number is on the list, you are invited back
